Hospital Records in China: What You Get

by China Medical Services

Hospital records are important for continuing care after a visit in China. Ask the hospital which documents are available for the services you receive and how to request them. A record-access guide should not promise that every institution supplies the same documents, release format or translation. Confirm the process for the specific hospital and the patient’s circumstances.

Ask Which Records Can Be Supplied

Create a document checklist with your home clinician or other intended recipient. Ask about consultation notes, test reports, original imaging, procedure documentation, discharge information and medication instructions where relevant. Keep the distinction between a written imaging report and the underlying image files. Record what was requested, supplied or still pending.

Questions to Confirm

  • Which documents will be issued for the appointment or admission?
  • What identification, permission or request form does this hospital ask for?
  • How can a family member obtain records if the patient wants them to assist?
  • Are original image files available, and how can the recipient confirm they open correctly?
  • Is an English document or translation available, and who is responsible for it?
  • Who can be contacted about a missing document after the visit?

Before the Next Commitment

Do not assume that a translation satisfies an insurer, receiving hospital or official authority. Ask each recipient about its own requirements. Preserve the original document with the translated version, including dates and issuing institution. If the two versions appear inconsistent, request clarification before using them to make a clinical or administrative decision.

Match the Request to the Recipient

A home clinician, an insurer and another reviewing hospital may ask for different documents. Identify the intended recipient before requesting a translation or additional copy. Ask the recipient what it requires and ask the issuing hospital which material it can supply. Neither a coordinator nor a general article can guarantee that a document satisfies every organization.

If you are requesting records remotely, ask the hospital how identity and permission are checked and whether a representative can assist. Confirm the process before sending personal documents. Do not assume that a particular passport, patient card, notarization or delivery method is universally required.

Keep a list of requested and received items. If a report refers to an image set or another attachment, ask whether that material is available separately. Check that electronic files can be opened by the intended recipient. If something is missing, record it as pending and identify the hospital contact responsible for the request. This helps organize the handover without pretending that a short discharge summary is the entire clinical record.

Coordination and Clinical Responsibilities

China Medical Services helps organize hospital enquiries, existing records, appointment communication and practical support. Clinical decisions belong to qualified treating professionals. Ask about the specific tasks you need through our patient coordination service; confirm availability, scope and fees in writing before proceeding.

For an enquiry, describe the question, the records already available and the support requested. Do not post medical files publicly. Ask how patient permission and secure document transfer will be handled before sending sensitive information. A coordination reply is not a clinical recommendation or a confirmation of hospital acceptance.

General planning information, not medical or legal advice. Individual clinical decisions belong to qualified treating professionals.
